As valued members and partners of National CooperativeRx, we strive to provide you the most transparent and up-to-date information regarding your pharmacy benefit plans. Therefore, we relay CVS Caremark formulary changes on a quarterly basis. As always, notifications were sent to members and their prescribers who are negatively affected by tier changes or drug exclusions.
Please ensure you are viewing the correct formulary list, as there are now core formularies and choice formularies. If you need help determining which formulary list(s) apply to your pharmacy benefit plan, please contact your National CooperativeRx account representative.
